To become a new patient please fill out all 5 new patient forms.
Once all 5 forms have been filled out and submitted we will reach out to schedule your first initial 30 minute clarity call with Dr. Ron Banuelos, L.Ac. This appointment will be held over either a Zoom call or a phone call.
After your first initial clarity call with Dr. Ron Banuelos we will either reach out to get you on the schedule for your new patient appointment (which can be in person or virtual), or we will reach out with a referral to a practitioner Dr. Banuelos feels is better suited to meet your needs.
Please note that we do not take insurance for sessions. We can supply a super bill for acupuncture appointments upon request at your visit as a courtesy.
